New Patients
Please arrive early so that we can complete your initial paperwork. Please bring the following
- Medical Insurance Card
- List of current prescription and over the counter medication, including dose and frequency
- Information about medial and surgical history
- Recent lab test results, x-ray, radiology or relevant records
Same Day Appointments
At Cascade Foot, we understand that accidents can happen at any time. If you experience an acute injury, have a wound, are dealing with an ingrown toenail, or suspect an infection, we're here to help. We offer same-day appointments to ensure you receive the care you need as quickly as possible. Contact us right away, and we'll do our best to see you the same day to address your concerns and get you on the path to recovery.

Insurance
Our office accepts a variety of HMOs, PPOs, Medicare, VA, and other health plans. Care Credit is not accepted. Please Call our office to verify acceptance of your insurance carrier
You are responsible for co-payments or charges that are not covered by your insurance. If you have any qeustions regarding billing or which insuracne plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Mask Policy
At CFC, we implement a mask policy to prioritize the health and safety of our patients, many of whom are immunocompromised and at heightened risk for infectious diseases. By requiring masks in our facility, we aim to create a safe environment that minimizes the spread of illness, ensuring that our vulnerable patients can receive care with confidence and peace of mind.
